HouseCalls

The JFS HouseCalls program provides vital in-home social work and healthcare services for the frail and homebound elderly who are living in senior housing or in their own homes in the community. Services are available to all seniors living throughout the MetroWest communities of Essex, Morris, and northern Union counties.

Administered in partnership with Overlook Medical Center, an affiliate of Atlantic Health System, the Jewish Community Housing Corporation and JCC MetroWest, the JFS HouseCalls program consists of an interdisciplinary treatment team consisting of:

Geriatric Nurse Practitioner staff from Overlook Hospital

Eldercare Social Work Services from Jewish Family Service of MetroWest.

Resident Management Staff from Jewish Community Housing Corporation

Senior Adult Program Staff from JCC MetroWest

HouseCalls services encompass the "wellness spectrum" and include the provision of in-home JFS eldercare social work services, Overlook Hospital geriatric nurse practitioner services, case management, health education, health screenings and referrals to medical transportation, home health aide and other support services. The HouseCalls staff team works collaboratively with the client's primary care physician, giving and receiving input on the individual's status and scheduling timely physician follow up appointments as necessary.

In addition to its in-home services, the HouseCalls program also promotes health awareness and wellness through free educational seminars held at the Jewish Community Housing Corporation senior residences. Topics have included: memory improvement techniques, adjusting to the changes of aging, medication safety, skin cancer prevention and screening, safety and falls prevention and "mastering the medical maze."

The HouseCalls program is funded with generous grant support from the Healthcare Foundation of New Jersey and a Community Development Block Grant from the Essex County Division of Housing and Community Development.
