WHO WE ARE
Coordinated Care of MetroWest (CCM) is a program that, after your incapacity or death, provides lifetime case management, through a trust you establish, for your family member with special needs or developmental and/or psychiatric disabilities.
• CCM is governed by a Board of Trustees and works in partnership with Jewish Family Service of MetroWest (JFS), the Jewish Community Foundation of MetroWest (JCF), and MetroWest ABLE.
• CCM has been a designated special needs community trust in the state of New Jersey since 1999.

WHAT WE DO
• When you can no longer care for your special needs family member,  Coordinated  Care of MetroWest (CCM), through case management, will oversee, coordinate and provide personal advocacy for the services, care, and benefits your family member requires, in accordance with a plan that you help create.
• CCM assists families in the planning process, helping them to create a care plan that reflects the needs of their family member, even as those needs may change.
• CCM will be there for your loved one to help in a crisis or for other unanticipated situations.
• CCM will communicate with designated professionals and family or friends about your loved one’s needs and changing situations.
• CCM will coordinate care provided by another similar agency, if your loved one chooses to live outside the MetroWest NJ area.
• Our aim is to help your loved one achieve his or her best and maintain a positive outlook on life.

HOW WE DO IT
• A case manager from JFS will oversee and monitor, on a regularly scheduled basis, the care plan and goals created specifically for your family member to ensure that needed services and benefits are in place and remain in place.
• The case manager maintains close communication with a designated relative or other appointed individual regarding the family member, and collaborates with social service providers, attorneys, and trust administrators, as needed, regarding the care plan and provision of services.

WHY CHOOSE CCM?
• Research shows that coordinating services for adults with special needs results in a better quality of life.
• CCM provides the opportunity to prepare a unique, individualized plan for the care of your family member.
• CCM assures you that care will be monitored and ongoing if you can no longer care for your family member.
• CCM can provide services on an interim basis (for instance, on a temporary basis during a caregiver’s lifetime) or through a will or estate plan.

OUR PARTNERS
Jewish Family Service of MetroWest (JFS) has been a social service provider in the community since 1861. JFS:
• Provides case management services.
• Helps families prepare a comprehensive care plan.
• Through the case worker, monitors and advocates for the needs and well-being of your loved one.
• Provides supportive counseling to ensure that your loved one leads a safe and fulfilling life.
• Has extensive human services expertise in working with adults with disabilities and the social services systems in New Jersey.

Jewish Community Foundation of MetroWest (JCF) was founded in 1949 and now is the largest community foundation in New Jersey. JCF:
• Houses nearly 700 component funds and foundations and has more than 60 years of experience managing charitable assets in donor-advised funds.

MetroWest ABLE was created by United Jewish Communities of MetroWest as a network of community leaders and professionals dedicated to advocating for
and supporting individuals and families with special needs. MetroWest ABLE:
• Provides education and raises awareness in the community about individuals with special needs and works with area families, synagogues, and local agencies to create a community that is accessible to all its members.
