Case Management

JFS provides case management services for adults with significant emotional and behavioral challenges. Our goal is to assist clients maintain the highest level of independence in the community, minimize hospitalizations and maintain stability in their everyday living. We accomplish this through a process of advocacy, monitoring, linkage, coordination and integration of all services.

JFS utilizes a strength-based, consumer-driven philosophy. We believe that clients and their families are the center of a team-based approach to decision making for all treatment and related services required. All treatment planning is individually tailored to meet the needs of the individual client and so no two plans will ever look alike. Clients and family members are actively encouraged to participate in collaborative planning so that there is a greater assurance of compliance and satisfaction with treatment choices.
 
 

Adult Case Management

For those adults who have had emotional, behavioral and psychiatric challenges, and who have had difficulty with past treatment service options, case management can assist in creating a more effective treatment plan. Effective case management is accomplished through thorough assessment of what has and has not worked for the client in the past, the client's current level of functioning and needs, and the client's present desired goals. We value the client's right to self-determination and self-management of their condition and treatment. Through the professional relationship formed between client and case manager, built on trust and mutual respect, the client is able to manage his/her mental health needs.
